About Our Driver Medicals

Professional driver medicals are a requirement for obtaining a licence to operate or work with commercial vehicles and heavy goods vehicles such as lorries and taxi’s.

Medicals are necessary for ensuring you are in good health to safely operate these vehicles, and the medical you receive will depend on which licence you are obtaining. All of our medicals are conducted by Dr A J Wallis, with over 20 years of GP experience and 10 years of experience with professional medical examinations.

Medicals For HGV/LGV Operation Licensing

In order to get your provisional HGV/LGV licence you will need to get a D4 Medical Examination Report filled in by a doctor to ensure you meet the DVLA Group 2 medical standards and then you will need to send it off with your D2 application form in order to get your provisional licence.

The D4 form is applicable for the following vehicle licenses and classes:

  • Vehicles over 3.5 (if your driving licence was obtained after 1997)
  • Vehicles over 7.5 tonne
  • Medium or Large Goods Vehicle (LGV) (C1, C1+E, C or C+E)

The D4 form must have been filled in within 4 months of your application to ensure that your health is still at the same level.

To obtain your D4 medical you will need to complete a questionnaire regarding your medical history and undergo a physical medical examination. The physical examination will involve an eyesight test and a blood pressure check.

To pass the eyesight test you will need to have a minimum requirement of 6/7.5 in one eye and 6/60 in the other, as measured on a 3-metre Snellen chart. If you normally require glasses or contact lenses, it is important that you bring these to the exam with you along with your current prescription.

Your blood pressure must not consistently exceed 179/99 in order to meet the DVLA Group 2 standard. This means your systolic blood pressure must not consistently exceed 180, or your diastolic blood pressure must not exceed 100 or higher to meet the requirement for this examination

The medical history questionnaire will require you to fill out details about your heath that may have an effect on your ability to drive. This will include information about various medical conditions such as epilepsy, seizures, insulin-treated diabetes, and other health issues. It is important to note that certain conditions will not affect your application if they are being managed correctly. It is a legal requirement to provide information that is correct.

The entire examination process typically takes around 15 minutes to complete. Our doctor will complete the D4 form based on the information you provide and the results of your physical examination.

The form contains sections specifically for your doctor to fill in, but you will be required to fill in Section 9 and Section 10 on page 8 of the D4 when you are with your doctor.

There is no specific YES/NO section on the form deciding whether you are fit to drive a passenger service vehicle as this is decided by the DVLA once they receive your forms. Please note that the doctor’s signature will be valid for only four months so send off your D4 form, D2 application form, and your driving licence as soon as you can.

Medicals For Public Service Vehicle Licensing

To obtain your provisional PSV/PCV license, you’ll be required to have a doctor complete a D4 Medical Examination Report to confirm your compliance with DVLA Group 2 medical criteria. Subsequently, you’ll need to submit this report along with your D2 application form to acquire your provisional license.

The D4 form is applicable for the following vehicle licenses and classes:

  • Passenger service vehicle (PSV) / passenger Carrying Vehicle (PCV) over 8 passenger seats (D1, D1+E, D or D+E)

The D4 form must have been filled in within 4 months of your application to ensure that your health is still at the same level.

Only your doctor should complete certain areas of this form. However, you’ll need to personally complete Section 9 and Section 10 found on page 8 of the D4 form while you’re with your doctor.

There isn’t a specific YES/NO section on the form to determine your fitness for driving a passenger service vehicle, as this determination is made by the DVLA after they receive your submitted forms. It’s important to be aware that the doctor’s signature will remain valid for only a span of four months. Therefore, it’s advisable to promptly submit your D4 form, D2 application form, and driving licence as soon as you’re able to.

Medicals For Heavy Machinery Licensing

Currently, there is no mandatory requirement from the Health and Safety Executive (HSE) to undergo a professional medical examination before obtaining a license for the use of heavy machinery such as forklifts, excavators, and other plant equipment. However, it is highly recommended that all plant operators undergo a medical to the standard of Group 2 drivers – particularly if you often operate in strenuous environments or with hazardous waste.

Some employers may request a medical from all employees who operate heavy machinery in order to maintain higher safety standards on site. It is recommended that you get in touch with one of our doctors using the form on this page to discuss the most appropriate medical examination for you.

Commercial Medical FAQs

The doctor will examine you in accordance with the requirements of the relevant D4 Medical Form.

During the Medical the Doctor will:

  • Measure your blood pressure
  • Perform two eye tests; the first before and the second after you put on driving glasses (this included in the medical fee). Do not wear contact lenses as these will have to be removed for the eye test. The eye test we be conducted using a Snellen chart.
  • Test your urine for Diabetes
    List on the D4 Form any medication you’re currently taking
  • Ask about your previous medical history (these questions are on the form and are compulsory).Once the medical is completed you post the form to the DVLA and they decide whether to renew your driving entitlements. If there is a reason why you shouldn’t be renewed the doctor will let you know at the time.

In order to become a lorry, bus or coach driver, you must:

  • Have a full car licence
  • Be over the age of 18 (there are some allowances)
  • Get a professional driving qualification, which is called the Driver Certificate of Professional Competence (CPC)
In order to get and maintain a full driver CPC you must:
  • Apply for a provisional lorry or bus licence
  • Pass the 4 tests that make up the Driver CPC to qualify
  • Take a total of 35 hours training every 5 years periodically to stay qualified
  • Sign a declaration every 5 years until the age of 45 to show the medical standards are still met
  • Provide a medical report every 5 years after the age of 45 to renew your driving licence. When you reach 65, this needs to be done every year.
The DVLA require existing lorry drivers to have a medical at the following intervals:
  • At the age of 45
    then every five years until the age of 65
  • After 65, the medical has to be completed every year
